Advancing health equity and addressing local population health outcomes through integrated care
We’re working to advance health equity by identifying and addressing system inequities, eliminating barriers to care, and supporting a modernized Medicaid program that brings together community-based organizations to address Social Determinants of Health and deliver culturally relevant care. Our highly coordinated services address physical and behavioral health needs, as well as social determinants of health such as food, clothing, shelter and transportation.
