The UnitedHealthcare Catalyst™ initiative aims to improve health equity and outcomes through community-centric partnership and action.
Communities across the country are taking part in the Catalyst process, which uses both clinical and local data to identify population health needs in select communities. UnitedHealthcare Community & State is convening organizations such as: Public Housing Authorities, Federally Qualified Health Centers and community and faith-based organizations in order to improve the health of communities. The long-term objective is to create measurable, sustainable and measurable changes that improve the community’s health and improve equity.
