The intersection of health and housing

The intersection of health and housing has come to a crossroads. UnitedHealthcare Community & State, Corporation for Supportive Housing (CSH) and Council for Large Public Housing Agencies (CLPHA) are coming together to share data and develop partnerships to better address the health needs of those living in publicly-assisted housing. The aim is to create an integrated system of collaboration and care to address community needs by aligning resources that identify and target emergent health needs, track gaps in care and support, and reform policies that address these gaps and improve health outcomes. The partnerships will result in direct and concrete improvements in the health and well-being of individuals, families, and communities that can be used as a model of how to do this successfully in communities across the country.
