Florida FQHC partners with local college to become medical home

Federally Qualified Health Centers (FQHCs) are designed to serve specific communities and understand the demographics and needs of those populations. This video with Community Health Centers, Inc. of Florida, shows how FQHCs can forge partnerships in unique and innovative ways. The FQHCs worked with a local college to become the students’ medical home. The educational institution served students with learning disabilities and the partnership with Community Health Centers helped make the scholars transition easier.
