Collaborating with Michigan providers to close gaps in care

 

In 2020, the Centers for Disease Control and Prevention reported that 32% of adults delayed preventive care, a decline even more significant among populations facing racial and socioeconomic disparities.1 To address this issue, UnitedHealthcare Community Plan of Michigan developed a targeted initiative focusing on member outreach and education. Collaborating with 282 primary care providers (PCPs), the plan sent cobranded letters to 9,699 members for Adults' Access to Preventive/Ambulatory Health Services (AAP) and 11,862 members for Child and Adolescent Well-Care Visits (WCV). This effort resulted in a 43% improvement in outreach for AAP and a 35% improvement for WCV among participating PCPs.

This year, UnitedHealthcare Community Plan of Michigan was honored to receive a Pinnacle Award in the “Improving Member Experience” category from the Michigan Association of Health Plans for its PCP cobranding initiative. Watch the video to learn more.
