Simplifying health care transitions for those with disabilities

 

Programs that help individuals with disabilities transition between health care and life phases are critical. That is because people with disabilities face issues as they make these changes throughout life. These changes include declines in health, loss of services and support, trauma, abuse and more. This policy brief, created by the National FQHC Advisory Board and National Advisory Board, outlines common health care challenges people with disabilities encounter as they age, as well as potential policies that could mitigate those obstacles.
