Primary care providers play a pivotal role in the care of patients receiving Medicaid. Access to quality primary care and preventive health services is associated with improved health outcomes and lower medical costs, but social drivers of health (SDOH) can impact access to care. Patients with the greatest risk for poor health outcomes may be disconnected from primary care. Connecting these patients to primary care improves health outcomes, reduces emergency department utilization and reduces total cost of care.
In its mission to help people live healthier lives, UnitedHealthcare Community Plan of Washington seeks to enhance the performance of the health care system and to collaborate with providers and partners to expand access to high-quality health care. UnitedHealthcare Community Plan of Washington initiated a program with Waymark in January 2023 using predictive technologies to provide community-based services and support to approximately 15,000 Medicaid patients in Washington State.
