Behavioral health

Medicaid members often face higher rates of behavioral health conditions due to socioeconomic factors and limited access to care. We take a whole person, trauma-informed approach to a range of behavioral health factors including social isolation and loneliness, opioid use disorder, and conditions and the unique needs of justice-involved populations.

 
 

Complex care focuses on providing comprehensive, coordinated care to individuals with significant health needs. This includes D-SNP members and those with long-term supports and services requirements due to aging, chronic illness or disability. By focusing on the whole person rather than individual conditions, complex care aims to deliver more personalized, patient-centered care.

 
 

Children and youth involved in foster care face some of the most complex challenges in the health care system. Our foster care clinical model is anchored in coordinated, integrated and trauma-informed care.

 
 

Federally Qualified Health Centers (FQHCs) are a cornerstone of care for Medicaid and underserved populations. They provide affordable and accessible care to millions of individuals each year, regardless of their ability to pay.

 
 

Health equity means everyone has a fair and just opportunity to achieve their full health potential and be as healthy as possible. However, many factors such race, ethnicity, gender, income, geography, sexual orientation, and disability status can influence a person’s health outcome. To achieve health equity, we must work to address individual, societal, and environmental barriers to health. 

 
 

Maternal health

We’re committed to supporting individuals throughout their maternal health journey, from before pregnancy to after childbirth. Our goal is to improve both the pregnant individual and child’s health outcomes by increasing access to care, considering whole-person health, and reducing health disparities.
